The State Physical Form must be used for: new entrants from out of state (must be within past year), all K, 6th and 10th grade physicals. State Physical Form

The Sports physical form MUST be completed for all athletes, including those having a 6th or 10th grade physical in addition to the form listed above. Sports Physical Form

The forms are NOT interchangeable with the exception of those done at Wildwood Pediatrics which uses a form meeting both requirements.
