Winter sports begin on December 2(Girls Basketball and Wrestling begin Nov.27).

All forms in this packet must be returned to your coach BEFORE the start of the winter season. Even if you played a fall sport these forms must be submitted before each season.

There will be a mandatory meeting for all student athletes and their parent/guardian on Wednesday, November 29 at 6:00pm in the auditorium. Individual team meetings will follow. All paperwork should be handed in before or at this meeting. Any questions about individual sports can be directed to the head coach. All coach's emails are found at the bottom of this page. If you attended the fall sports meeting this year you do not need to attend the meeting at 6:00pm. You may report directly to the team meeting at 6:30.
**Gymnastics & Ice Hockey will not have indivdual team meetings on this night.**

The CIAC requires that a valid sports physical be provided before a prospective athlete can practice and/or compete for any CIAC member school. All student athletes must have a valid sport physical on file with the school nurse BEFORE practice or try outs begin. Students will not be notified if their physical is expired, this is the responsibility of the student to make sure they are cleared with a valid physical.

Haddam Killingworth Athletics are governed by the Connecticut Interscholastic Athletic Association who set rules including but not limited to State Tournament play and qualifications; beginning and end of season dates; age and academic requirements; rules pertaining to residence and school transfers; to name a few.

In order to participate at any level several forms must be returned the nurse and coach BEFORE any student athlete can participate.

  1. Updated Sport Physical- given to the school nurse. The CIAC requires that a valid sports physical be provided before a prospective athlete can practice and/or compete for any CIAC member school. A valid physical lasts for 13 months. Physical appointments usually require significant advance notice, we urge any parent anticipating that their child may participate in sports, make their appointments far in advance. Extension notes from a physician will not be accepted. The student MUST have a current physical.

  2. Student & Parent Athletic Handbook Signature Page- signed by both the student and parent. At the start of the season, students and parents must review the hand book found which can be found on the school website. Hard copies are available on request. Please read through this handbook carefully as it has important information on eligibility and procedures and has recently been updated to include our new drug and alcohol policy. In order to participate in high school athletics students and parents must sign off stating that they understand these rules and regulations.

  3. Athletic Emergency Information Card- given to the coach. This form is very similar to the Emergency information form that goes to the School Nurse. It is for the Coach and Trainer in case an emergency arises during practice or games. This form is vital to the trainer and/or the coach to provide proper care.

  4. Student/Parent Concussion Education & Consent Form- signed by both the student and parent and given to the coach.

  5. Student/Parent Sudden Cardiac Arrest Education & Consent Form- signed by both the student and parent and given to the coach

Return to Play Concussion Protocol: This is the district’s return to play policy. Any questions can be directed to the school nurse Linda Kauffman (lkauffman@rsd17.org) or the Athletic Director.


Parent Request For Transport form: Athletes are expected to travel with the team to and from an athletic contest. Exceptions are EXTREMELY RARE. Some coaches do allow the athletes to be taken home from the contest site by parents or guardians if they have this form in their possession.

Parent Request For Transport By Another Party form: If you anticipate that someone other than a parent or guardian will be bringing your student home, they must use this form which must be delivered to the coach before the contest. If there is someone whom you approve for the season, the form should then be given to the Coach at the beginning of the season.

Regional School District 17 has insurance coverage for Athletes. This can take over if and when personal insurance is used up. If an athlete is injured, you will be contacted by the school nurse as to what you will need to do to make the insurance available if needed. You may also purchase a policy pertaining specifically to athletics, but that is a matter of choice. If your child is injured at a practice or game the coach MUST fill out an incident/accident report and submit it to the school nurse within 48 hours of the incident.

Coaches may also have a set of guidelines and rules specific to their team. While these rules may not supersede the rules of the CIAC or RSD 17, they are a set of rules that parents and athletes are expected to understand and follow.
